Your primary objective will be to assess, plan, implement, and evaluate individualized nursing care plans that promote patient health, recovery, and well-being. You will serve as a vital advocate for patients, ensuring their needs and preferences are respected while providing education to empower them and their families in managing health conditions. This role requires close collaboration with interdisciplinary teams to coordinate care and monitor patient progress, particularly in managing complex medical and surgical cases. Ultimately, your work will contribute to improving patient outcomes and enhancing the quality of life for individuals receiving home health care services. Psych experience a plus.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of California.
