Key Responsibilities
  • Conduct thorough psychosocial assessments for individuals, adolescents, families, and groups, identifying underlying issues and determining appropriate intervention strategies.
  • Collaborate with multidisciplinary teams to develop and implement personalized treatment plans based on assessment findings, ensuring a holistic approach to care.
  • Maintain effective communication with both staff and clients, documenting treatment plans and updates during regular interdisciplinary meetings and patient rounds.
  • Deliver evidence-based therapeutic interventions, including Cognitive Behavioral Therapy (CBT) and other clinical modalities, tailored to meet the unique needs of each patient or family.
  • Provide case management for assigned patients, facilitating collaboration with external professionals and community resources to ensure comprehensive care.
  • Ensure timely and accurate completion of all documentation in adherence to departmental standards and regulations.
  • Support clients in achieving their treatment goals by offering continuous encouragement and resources as they work through their individualized service plans.
Patient Population / Client Focus

Your role as a Mental Health Counselor will involve working primarily with individuals facing a range of mental health issues within the hospice care context. This position requires a strong understanding of the complexities of grief and loss, and the ability to provide support to clients dealing with terminal illnesses and end-of-life challenges. Qualifications
