Position Overview

Job Summary
The Clinical Coder extracts clinical information from a variety of medical records and assigns appropriate procedural and diagnostic codes to resident records. Accurately assigns and sequences ICD-10-CM diagnostic and procedural codes and/or E&M and procedural codes to patient records for use in reimbursement and data collection.

Roles and Responsibilities
• Confirms appropriate diagnosis related group assignments.
• Verifies patient information to identify any documentation vs. report discrepancies and to ensure codes and other abstracted data are accurately applied to appropriate patient's account/encounter.
• Codes and abstracts medical records to provide information for financial reimbursement and data collection, converts interpreted data into appropriate code numbers.
• Assess documentation and/or queries physician for additional information when indicated to clarify or provide specificity to a diagnosis, symptom, or reason for a service.
• Utilizes various classification systems to code and categorize patient information for insurance reimbursement purposes, for databases and registries, and to maintain resident medical and treatment histories.
• Analyzes information and enters into computer system.
• Retrieves patient medical records from medical personnel from various institutions.
• Protects the security of medical records to ensure that confidentiality is maintained and adheres to all HIPAA guidelines and regulations.
• Verifies signatures and reviews records for completeness, accuracy, and compliance with regulations.
• Attends and participates in annual company required in-service training programs.
• Other duties as assigned.

Qualifications
Education: High School / GED
Experience: 1-3 years

Licenses and Certifications
Certified Medical Coder with one of the following active certifications (CPC from AAPC; or CCA, CCSP, CCS from AHIMA) preferred or; Licensed Practical Nurse or; Certified Medical Assistant or; Certified Nursing Assistant

Physical Requirements
Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs. Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus.
