Westminster at Home is seeking an RN Clinical Team Leader to join our team for Home Health and Hospice. Hours for this position are Monday - Friday 8:30 AM to 5 PM; On-call hours are required based on business demands, which can result in weekend hours. The full time position offers a robust benefits package with medical, dental, vision, 403(b) retirement, employer match, paid time off, and paid basic life insurance along with a cell phone stipend.

The RN Clinical Team Leader is responsible for utilizing a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required to meet clients' health needs. The Clinical Team Leader coordinates available resources to promote quality and cost-effective outcomes, assuring compliance with corporate standards and federal, state, and local regulations. The Clinical Team Leader completes ongoing audits of charts and computer files to continually improve data quality and demonstrates knowledge and support the philosophy, purpose, goals, and policies.

Requirements

  • Registered Nurse license issued by the Virginia Board of Nursing; BSN preferred but not required.
  • Current CPR certification (Basic Life Support).
  • Minimum of one year of acute care medical-surgical nursing experience and one year of experience in a Medicare-certified home health agency. 
  • Prior supervisory experience preferred.
  • Basic computer literacy with experience using Windows-based programs preferred.
