Position Summary

 

At Stony Brook Medicine, a TH Staff Associate/Supervising Case Manager is a valuable member of our team, who provides clinical services to our patient population.

Qualified candidates will demonstrate superior patient care and possess outstanding communication skills while adhering to our high standard of excellence.

Duties of a TH Staff Associate/Supervising Case Manager in the Care Management Department may include the following but are not limited to:

 

Brief Description of Duties: 

o   Support efficient use of resources.

o   Input into department budget, utilization review, staffing and department systems and process.

o   Provide direction and guidance to staff regarding role and responsibility.

o   Oversees staff performance including Case Managers and Appeal Managers.

o   Input in performance on other disciplines within the department.

o   Support the financial goals and strategic plans of the hospital.

o   Maintains knowledge and competence in Care Management programs, performance improvement activities, staffing analysis, weekend activities and data collection as directed by the department

o   Maintain a high level of customer service.

o   All unit coverage as indicated.

o   All other duties as assigned.

o   Serves as a resource for interdisciplinary staff by providing guidance and education on case management processes, regulatory requirements, and care coordination best practices.

o   Participates in the development and delivery of educational programs and in-service training related to case management, discharge planning, and patient-centered care.

o   Act as a mentor to staff and coordinates department orientation of new staff.

Qualifications

 

Required Qualifications: NY State RN License and Bachelor Degree.  Five years acute care experience.  Experience in CM or Managed Care.  Working knowledge of observation, RAC, Cert, MAC.  Understanding of discharge planning and barriers in the system to discharge.  Experience with Unit Rounds and transitional care information needed.  Interview skills/high level CSR service with demonstrated leadership experience.  PRI Certified.  Weekday and weekend flexibility, day and evening rotation as needed.

 

Preferred Qualifications: Master Degree.  Certified in CM/Quality/CDI.  Recent tertiary care experience.  Experience in data base systems e.g. excel, word, access and hospital systems.  Working knowledge of MCG/Interqual criteria.  Track, trend and analyze reports with documentation.  Managed Care or IPRO experience.  Knowledge of regulatory requirements i.e. CMS, OBS, Two Midnight Stay, EMTALA, etc.  Experience in Peds, NICU, ED or Psychiatry.  Bi-lingual.

Anticipated Pay Range:

The salary range (or hiring range) for this position is $107,100 - $152,698 Base

The above salary range represents SBUH's good faith and reasonable estimate of the range of possible compensation at the time of posting.  The specific salary offer will be based on the candidate's validated years of comparable experience.  Any efforts to inflate or misrepresent experience are grounds for disqualification from the application process or termination of employment if hired.

Some positions offer annual supplemental pay such as:

  • Location pay for UUP, CSEA & PEF full-time positions ($4000)
