Your future role at a glance

Location: Appleton, WI

Facility: Ascension St Elizabeth

Department/Specialty: Anticoagulation

Schedule: PRN | Days Could work between 8 am - 5pm, Monday - Friday

How you'll make an impact in this role
  • Deliver compassionate, high-quality direct patient care that adheres to all established protocols and guidelines.
  • Implement and continuously monitor comprehensive patient care plans to ensure the best possible health outcomes.
  • Document and communicate critical patient condition changes swiftly, enabling timely and informed decisions by the healthcare team.
  • Coordinate interdisciplinary care, acting as the primary point of contact for seamless and well-coordinated patient treatment.
  • Lead the assessment and planning of patient discharge needs to ensure a smooth transition and continuity of care.
What minimum qualifications you'll need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • One or more of the following required:
    • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State. Licensure is dependent on state associate works in.
    • Registered Nurse credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date. Licensure is dependent on state associate works in.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
