Description

A&B Homecare Solutions, a Modivcare Personal Care Service, is looking for a Bilingual Care Coordinator Associate. You will have the opportunity to make a positive impact on the lives of our patients by ensuring their care needs are met effectively and efficiently.

  • Pay: $20.00-$23.00/Hourly based on experience 
  • Schedule: M-F 8:00AM-5:00PM (Rotating on call required)

You will...

  • Assign field staff schedules and confirm assignments with employees and patients.
  • Speak with patients or patient families regarding schedules, call-offs, cancellations, and appointments.
  • Document client data and visits in the scheduling system
  • Be the backup to the Intake department for receiving and coordinating home care service requests.
  • Participate in office's on-call rotations to provide after-hours support.
  • Understand that communication is vital within the office as well as to everyone involved in our clients' care.

  • High school diploma or equivalent.
  • 1+ years of customer service experience required.
  • 1+ years of home healthcare experience preferred.
  • Experience using HHAeXchange preferred.
  • Bilingual in English & Spanish required.
