Job description

Job Summary 

Provide timely and effective medical social work and discharge planning that encompasses initial and continuing psychosocial assessment of individuals for their medical, financial, emotional, and spiritual needs.  This assessment involves the ability to counsel, interview, evaluate, organize, prioritize, communicate, document, and implement a safe comprehensive cost effective discharge plan.  The plan includes referrals to appropriate healthcare providers and community resources for continuity of care and to ease the patient's transition through the continuum of care. Provides coordination of healthcare services for patients to meet healthcare requirements in a cost-effective manner.  Participates in a collaborative process with the interdisciplinary team to assess, plan, implement, coordinate, monitor and evaluate options and services to meet individual's health care needs while promoting quality, cost-effective outcomes.  Participates in the utilization review process for patients. Demonstrates knowledge of local, state, and federal regulations and guidelines in accordance with policies, procedures, mission and objectives

    • Education:Masters Degree in Social Work from a school accredited or approved by the Council on Social Work or Masters Degree in Counseling or Rehabilitation.
    • Experience: One year experience
  • Preferred Requirements
    • Education:Master's Degree in Social Work/Counseling
    • Experience:2 years recent acute health care experience. Behavioral health experience

  • Assess psychosocial needs of individuals for referral to appropriate healthcare providers.
  • Expedite timely processing of referral management.
  • Complete electronic documentation - assessment form and progress notes.
  • Complete d/c arrangements on referred cases.
  • Provide crisis intervention and support to patients and family.
  • Provide assistance with ALTCS/AHCCCS applications.
  • Provide assistance with domestic violence and substance abuse issues.
  • Provide assistance with advance directives.
  • Provide assistance to patients and families coping with chronic/terminal illness.
  • Develop and help implement avoidable day/readmit OFI action plans.
  • Maintain knowledge of community resources, available resources, and CHN Policies.
  • Respond to internal and external requests in a professional and timely manner. Maintain confidentiality.
  • Advise and counsel as appropriate.
  • Work with healthcare team in development of treatment plans for their select group of patients.
  • Act as resource person for other members of healthcare team. Assist in orientation and in-service training.
  • Participate in planning and implementing quality improvement initiatives.
