Position Summary

This is a professional OPS position with the responsibility of a Questionnaire for Situational Information (QSI) Assessor who conducts evidence-based assessments for the Agency for Persons with Disabilities using the Questionnaire for Situational Information for all APD eligible persons and conduct statewide productivity quality assurance procedures to ensure systematic monitoring and evaluation of programs to ensure that standards of quality are being met.

The Work You Will Do 

Analyze statewide QSI data and ad hoc reports to assess QSI productivity, providing technical assistance and conduct quality assurance activities to enhance performance levels.

Assist and attend quarterly region-specific calls and/or statewide quality assurance conference calls with statewide QSI team, including assisting with preparation of meeting logistics. Assist with managing the assessment workflow statewide including priority Agency assigning assessments, reviewing assessment reports, tracking completion, and submitting reports to the Region and State office, as requested.

Successfully complete assessments using the QSI, which includes review of the person's central record including recent assessments, progress notes from medical records, school records, previous support plans, and relevant information from other sources as appropriate; conduct of a face-to-face interview with the person with a developmental disability and the person's guardian and family, and interviews the person's caregivers, health care personnel, Waiver or Pre-Enrollment support coordinator or service providers as appropriate; verification of the person's demographic information; and validation of information and scores before entering the results into the QSI online application.

Refer persons and family members to their Waiver or Pre-enrollment support coordinator for provider information and available resources in the community, based on needs identified during the administration of the QSI which also includes providing customer service in response to agency needs and represent the QSI process to Waiver or Pre-enrollment support coordinators, providers, and outside agencies.

Recertify annually as a QSI Assessor to demonstrate inter-rater reliability and interview competencies on the QSI protocol.

Perform other duties as assigned, including administrative tasks.

Minimum Qualifications

  • Valid Driver's License or other efficient means of transportation to travel for work purposes.
  • Must have a bachelor's degree from a college or university and a minimum of four (4) years professional experience working with persons with developmental disabilities.
  • Must be or eligible to become a certified QSI Assessor and successfully complete all aspects of QSI initial training and on-the-job orientation as a new QSI Assessor.
  • Must demonstrate excellent communication skills used in conducting person-centered interviews and sufficient computer skills to access and use a web-based assessment system to obtain demographic and other details specific to locating persons and to record results of assessments.

Knowledge, Skills, And Abilities

Knowledge of:

  • QSI protocol: its purpose, ability to score and interpret QSI items correctly.
  • Person-centered process used in conducting interviews for the QSI.
  • APD processes and procedures, including i-Budget/ Waiver and pre-enrollment.

Ability to:

  • Use computer-based systems, including the APD intranet: iConnect, SharePoint, Microsoft Office, Microsoft Teams, and Excel spreadsheets.
  • Communicate effectively in face-to-face, electronic, and written formats.
  • Establish and maintain effective working relationships with persons, families, providers, and staff within the region and statewide.
  • Prepare reports.
  • Manage, organize, analyze, and monitor data related to the QSI.
  • Administer APD approved functional needs assessments.
  • Travel for work purposes.

Direct Deposit Program

As a condition of employment, a person appointed to a position in the State Personnel System is required to participate in the Direct Deposit Program.  Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. Retirement funds are also required to be in the Direct Deposit Program.

Background Screening Requirement

It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.

No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.
Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement, and Federal criminal records checks through the Federal Bureau of Investigation and may include local criminal records checks through local law enforcement agencies.

Pursuant to S. 110.1127(1), Florida Statutes, this position requires a background investigation, including fingerprinting.

APD only hires U.S. citizens, and those lawfully authorized to work in the U.S. APD participates in the U.S. government's employment eligibility verification program (E-Verify). E-Verify is a program that electronically confirms an employee's eligibility to work in the U.S. after completing the employment eligibility verification form (I-9).
