Within the context of a client and family centred care model, this position functions under the operational supervision of the Manager or Team Leader the practice supervision of the PT Practice Coordinator and the direction of a licensed designated physiotherapist to work as a member of the health care team. Provides assigned physical therapy therapeutic interventions; assists in modification of PT care plans in order to enable clients to maximize potential for independence. Provides functional direction to client and family, reports relevant observations of client's condition and progress in writing or by utilizing basic computer literacy, manages equipment safety and inventory.

ABOUT HOME HEALTH PROGRAMS:

Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.

THE CLIENT POPULATION:

Home Health clients typically have on-going health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence. Three Bridges serves the Downtown, West End, Coal Harbour, Yaletown and Fairview Slopes areas.

Education & Experience

• Completion of a recognized Diploma level Rehabilitation Assistant Program with Physical therapy components plus one (1) year's recent, related experience working with target population or an equivalent combination of education, training and experience.
• Valid BC Driver's License and access to a vehicle. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.
As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
