Job description

BASIC FUNCTION:

Under the supervision of the Team Leader, performs duties for the care, treatment and safety of the individuals with developmental disabilities, provides required personal care services and carries out procedures and techniques geared towards a treatment, development and rehabilitation, carries out activity therapy programs by participating with individuals in recreational and social activities, and carries out any and all lesson plans given by supervisors for the day.

DESCRIPTION OF DUTIES AND RESPONSIBILITIES:

  • In compliance with Milestone, Inc.’s Philosophy and policies regarding human
  • rights; intervenes on behalf of the individual(s) served and reports to the appropriate agency (OIG, DCFS, IDPH) and/or supervisor. 
  • Reliable, prompt attendance and present for shift change report, demonstrating flexibility and cooperation with team members as needed to provide adequate facility coverage and follows self-scheduling guidelines.
  • Monitors the individuals’ whereabouts continuously and records changes/problems, accordingly contacting appropriate supervisors when necessary.
  • Participates actively in various assignments necessary, providing on-going active treatment for all assigned individuals according to each individuals plan, daily programs and lesson plans. Maintains accurate, up-to-date and orderly individual records for all individual programs and/or behavior change.
  • Accurately reports and completes data in a prompt manner.
  • Accurately reports, records and completes data of specific occurrences such as seizures, toileting, intake, output, menses, weight, injuries and/or medical emergencies and contacts supervising and/or nursing personnel when appropriate or needed in a prompt manner. Accurately fills out injury report when needed.
  • Monitors all individuals’ health needs, contacting supervising personnel and nurses in a prompt manner.
  • Motivates and encourages individuals in order to promote their development and treatment, participates in the implementation of established treatment and development programs; observes and reports behavior and reactions.
  • Accurately communicates shift information in communication log and in verbal report to on-coming staff. Relates any specific problems of behavior, health or building maintenance. Reads and signs off on any memos each shift.
  • According to established procedures, documents and reports any specific problems of behavior, health or building maintenance.
  • Implements Individual Service Plans, assisting individuals with development of daily living skills such as cleanliness, personal hygiene, table manners and social skills; reinforcing behavior development programs where in effect, maintaining active treatment on a continuous basis per individual plans and assisting individuals in completing the daily lesson plan.
  • Display knowledge of purpose of programs, activities on daily lesson plan and active
  • treatment.
  •  Accurately reports, records and completes charting for individual=s phone calls, visitors, and mail.
  • Ensures for individuals safety during emergency situation and demonstrates knowledge of proper disaster procedures, use of fire extinguishers and alarm systems, door alarm system and available emergency exits.
  • Assists in maintaining physical plant in an exceptionally clean, safe and pleasant condition. Reports problems and/or concerns according to established procedure.
  • Appropriately uses cleaning and sanitation supplies in completing spot cleaning in a
  • prompt manner.
  • Works in cooperation with laundry/housekeeping staff in keeping individuals clothing and household linens in clean, well-fitting and safe condition. Reports needed mending and repairs in a prompt manner.
  • Ensures individuals have their own clothing on and in their rooms.
  • Assists with safe transportation of individuals to and from recreational and/or day program activities (if authorized to drive Milestone vehicles).
  • Provides appropriate supervision and assistance to individuals while in the community. Assisting individuals in any way needed, helping them to participate in the outing to the individual’s fullest abilities, while promoting a positive image of the individuals and the agency to the general population.
  • Actively looks for work throughout the shift in any setting assigned, communicating with team leader and/or supervisor when assignments are completed.
  • Establishes and maintains cooperative relationship with peers and supervisors, promoting good communication and positive problem-solving.
  • Maintains acceptable standards of confidentiality concerning individuals, peers and the agency.
  • Uses all mechanical lifts, belts, chairs and other appliances appropriately and cleans them as necessary. This includes all wheelchairs.
  • Responsible for knowledge of individual diets following the established menu plan and adhering to prescribed diets.
  • Maintains cordial and cooperative relationship with individuals served, their families and guardians and other staff members, utilizing and promoting positive problem solving skills and ensuring a positive, pleasant environment.
  • Participates in team meetings 2 times per month cooperating with team during scheduling to ensure adequate coverage keeping overtime to a minimum.
  • Complies with all Milestone, Inc. policies and procedures.
  • Performs other duties as directed or assigned as necessary to provide for appropriate care for the individuals.

QUALIFICATIONS:

  • Must be over 18 years of age.
  • High school diploma or GED.
  • Must pass the TABE test.
  • Must be able to intervene physically in behavioral and health related crisis with
  • individuals.
  • Must complete habilitation aide certification.
  • Nurse’s aid experience or some personal care experience preferred.
