Summary:


Under the supervision of the Housing Initiatives Supervisor, the Migrant Case Manager is responsible for supporting migrant families in Monroe County by ensuring they are welcomed into the community, informed about available resources and services, and connected to essential support needed to achieve stability and self-sufficiency. The Migrant Case Manager will work closely with community-based organizations, service providers, landlords, schools, healthcare systems, and other community partners to help families access safe and affordable housing, healthcare, education, employment resources, transportation, financial assistance, and other critical services. The position also assists families in navigating life in the Rochester area while addressing immediate needs and promoting long-term community integration and well-being.


Essential Functions:

  • Reports to the Housing Initiatives Supervisor.
  • Support migrant families arriving in Monroe County by providing a positive introduction to the community and available resources.
  • Assist migrant families in securing safe, sanitary, stable, and affordable housing within the Rochester and Monroe County area.
  • Coordinate emergency and supportive financial assistance to address immediate family needs, including transportation, food, clothing, household items, and other essential resources.
  • Connect families to community-based services and support, including healthcare, mental health services, public benefits, schools, childcare, employment programs, legal services, and language access resources.
  • Provide ongoing case management services to support family stabilization, community integration, and progress toward self-sufficiency.
  • Coordinate housing-related assistance, including rental subsidies, security deposits, first month's rent, utility assistance, and landlord communication as applicable.
  • Assist families with navigating immigration-related appointments, applications, notices, and referrals to appropriate legal or advocacy services when needed.
  • Conduct community orientations to educate families on transportation, school systems, healthcare access, tenant rights and responsibilities, public safety, and available community resources.
  • Support participants in obtaining employment, vocational training, educational opportunities, and workforce development services to encourage long-term stability and independence.
  • Maintain accurate and timely case notes, documentation, service plans, referrals, and reporting requirements in accordance with program standards and funding guidelines.
  • Ensure consistent compliance with agency policies, procedures, and regulatory requirements.
  • Maintain regular communication and follow-up with assigned families, including home visits, housing stability assessments, and wellness checks as required.
  • Advocate on behalf of migrant families to reduce barriers related to housing, language access, transportation, healthcare, and access to community resources.
  • Serve as a cultural and linguistic liaison between migrant families, service providers, landlords, schools, volunteers, faith-based organizations, and community partners.
  • Participate in community meetings, collaborative partnerships, outreach efforts, and interagency coordination to strengthen services available to migrant families in Monroe County.
  • Maintain flexibility to support occasional evening and weekend activities, appointments, or emergency family needs.
  • Perform other duties as assigned by the Ibero-American Action League President & CEO or designee.??

This position holds some degree of independence within the framework of the agency's policies and procedures. The individual must be resourceful and able to display initiative and understanding of management systems and interpersonal relations. Must keep immediate supervisor abreast of issues and concerns which merit intervention from higher level management staff.

  • Diploma or GED preferred.
  • A minimum of 2 years' experience engaging in community-based services.
  • Valid driver's license, ability to provide your own transportation and travel to various locations.
  • Experience working with community human services providers.
  • Experience working with migrant populations preferred.
  • Bilingual in English and Spanish required.
  • Must be able to perform all functions with minimal supervision.
