Aspire Home Healthcare is seeking a compassionate and skilled Registered Nurse (RN) to join our growing team in the Grass Valley area. This part-time position offers flexibility, autonomy, and the opportunity to make a meaningful impact in the lives of patients by providing high-quality care in the comfort of their homes.

Position Summary

The Registered Nurse is responsible for conducting patient assessments, developing and implementing individualized care plans, coordinating services, and providing skilled nursing care in accordance with physician orders and agency policies.

Responsibilities
  • Perform comprehensive patient assessments and ongoing evaluations
  • Develop, implement, and update individualized plans of care
  • Administer medications and treatments as prescribed
  • Provide patient and family education regarding disease management, medications, and safety
  • Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members
  • Complete accurate and timely clinical documentation
  • Coordinate care with therapists, home health aides, physicians, and caregivers
  • Ensure compliance with state, federal, and agency regulations
Qualifications
  • Current and unrestricted California Registered Nurse (RN) license
  • Minimum one year of nursing experience required
  • Home Health experience preferred
  • Strong assessment and clinical skills
  • Knowledge of Medicare home health regulations preferred
  • Current CPR certification
  • Valid California driver's license and reliable transportation
  • Excellent communication, organization, and time-management skills
