As a Nurse Practitioner, you will play a vital role in our integrated care continuum, providing exceptional medical services to patients with a focus on behavioral health, addiction medicine, and primary care. This is an exciting opportunity to work with our multidisciplinary team to deliver exceptional care and ensure our patients receive comprehensive treatment for their mental health, addiction, and primary care needs.

Responsibilities:

ยท Conduct comprehensive medical assessments to determine appropriate treatment plans

ยท Develop and implement individualized treatment plans in collaboration with the patient and the care team

ยท Prescribe medications and monitor their effectiveness and side effects

ยท Provide medication management and adjustments as needed

ยท Collaborate with other healthcare professionals to ensure coordinated and integrated care

ยท Educate patients and families on preventive care measures, treatment options, and healthy lifestyle choices.

ยท Monitor and evaluate patient progress, adjusting treatment plans as necessary

ยท Conduct regular follow-ups and monitor patient progress

ยท Maintain accurate and up-to-date medical records and documentation

ยท Stay informed about the latest research and advancements in integrated care

Qualifications:

ยทย Valid APRN license in Florida.

ยท Master's degree in Nursing or Post MS Certificate from an accredited University/College.

ยท Board Certification as a Family Nurse Practitioner (FNP), Psychiatric Mental Health Nurse Practitioner (PMHNP), or Adult Gerontological Primary Care Nurse Practitioner (AGPC).

ยท 2 years of experience as a Nurse Practitioner (preferred). Willing to work with motivated new graduates.

ยท Excellent clinical assessment, diagnostic, and treatment skills.

ยท Ability to work collaboratively in a multidisciplinary team environment.

ยท Exceptional interpersonal and communication skills, with a patient-centered approach.

ยท Proficient in using electronic medical record (EMR) systems and other relevant healthcare software.
