Home Health RN Case Manager – Fort Worth / Tarrant County, TX

We are seeking an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position is best suited for a clinically strong Registered Nurse with recent home health case management experience, strong assessment skills, solid documentation habits, and the ability to manage an assigned patient caseload independently in the field.

Key Responsibilities

  • Manage an assigned home health patient caseload in the field.
  • Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
  • Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
  • Complete accurate, timely clinical documentation, including OASIS and visit notes.
  • Monitor patient progress, identify changes in condition, and communicate updates appropriately.
  • Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
  • Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.

  • Active Texas RN license in good standing.
