Job description

Job Summary:

The role of a CHHUNY High Needs Care Manager is to be the key point of contact in managing children & youth services. The Care Manager will be the center point of the communication to ensure all providers are working together with the child/youth’s best interest in mind. The Care Manager will conduct comprehensive assessments in order to obtain the most accurate information to build off of the identified needs and risk factors.

Duties:

  1. Responsible for, but not limited to comprehensive assessments, outreach and engagement, service and treatment linkage and coordination
  2. Comprehensive client centered care planning development and management with the child/youth and parent/guardian to assist in the integration of medical and behavioral health services, and social health services
  3. Coordination of care with community service providers that are part of the client care team
  4. Assessment of psychosocial supports and linkages with medical, dental and behavioral health care providers, as well as, educational, employment, transportation community resources
  5. Build and use effective communications strategies among peers, medical staff, addiction and mental health providers and other community agencies through the use of electronic assisted devices including Telehealth and other interactive technology
  6. Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators/performance measures, patient satisfaction, and plan adherence
  7. Participate actively in team meeting and supervision to enhance in professional development.
  8. Maintain excellent record keeping.
  9. Use of personal vehicle for work and potential transportation of clients
  10. Provide care management to children/youth in the seven counties we cover 
  11. On-Call duties as assigned for nights and weekends
 

Minimum Qualifications: Health Home Care Managers that serve High acuity enrollees must have the following minimum education and experience:

• A Bachelors of Arts or Science with 2 years of relevant experience, or

• A License as a Registered Nurse with 2 years of relevant experience, or

• A Masters with one year of relevant experience.

Care Management Agencies may seek a waiver from the State for care managers that have demonstrated experience but do not meet the required criteria (process to be determined by DOH).

Other Skills/Knowledge and Experience

  1. Demonstrates ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers
  2. Knowledge of local medical and behavioral health services, and additional community resources/supports
  3. Ability to multi-task in a fast-paced environment, have good problem-solving skills, as well as excellent time management and organizational skills and the ability to remain calm in a crisis while providing crisis intervention
  4. Must be able to type 30 WPM
  5. Proficient in Internet navigation, Microsoft Office (i.e. Word & Excel, Microsoft Outlook)
  6. Proficient in the use of electronic health records
  7. Comfort working in the community

    Licensure/Certifications

    New York State driver’s license, safe driving record
