Registered Nurse (RN) Case Manager - Wichita - PRN Weekend

Job Summary:

We are seeking a compassionate Weekend Registered Nurse (RN) to join our team as a Case Manager in Wichita, Kansas. As a RN Case Manager with Specialized Home Care, you will work one-on-one with patients in their home environment to establish and oversee all aspects of case-managing patient's care. This role requires strong clinical skills, excellent communication, and the ability to work independently.

Responsibilities:

  • Assume responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care.

  • Collaborate with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.

  • Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.

  • Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.

  • Participate in on-call rotation as reasonably assigned.

Qualifications:

  • Minimum of 1 year of nursing experience with a preference for 2+ years in an acute or post-acute care setting.

  • Active State Registered Nurse licensure is required.

  • Proof of current CPR certification and valid Driver's license.
