Key Responsibilities
  • Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial concerns, acting as a critical member of the primary care team.
  • Conduct comprehensive patient intakes, focusing on diagnostic evaluations that inform treatment goals and plans tailored to individual needs.
  • Provide focused, short-term interventions for clients requiring immediate mental health support while facilitating longer-term solutions as needed.
  • Advise primary care providers regarding patient referrals, ensuring seamless transitions to specialized mental health services when necessary.
  • Initiate follow-up communication to assess patient progress and make adjustments to treatment strategies as required.
  • Develop and implement relapse prevention plans to aid patients in sustaining their mental health gains.
