You will work in a fully remote environment, which allows for flexible scheduling while ensuring you provide ongoing support to our patients at the hospice center. Our approach emphasizes collaboration among healthcare professionals, including physicians, therapists, and nursing staff, to create a cohesive unit that champions patient-centered care. This role may require you to attend occasional team meetings or training sessions in person.

• Conduct comprehensive psychosocial assessments through structured interviews, careful observations, and detailed examination of medical records to fully understand the myriad emotional, social, and environmental factors influencing each patient's recovery trajectory.
• Provide empathetic counseling and guidance to patients and their families, facilitating discussions around adjustment to illness, grief, and emotional coping strategies tailored to individual circumstances.
• Employ collaboration with interdisciplinary teams to develop, implement, and monitor customized treatment plans that address the complex psychosocial needs of patients.
• Assist patients in navigating community resources, connecting them to essential support groups, vocational retraining services, or housing assistance that might be beneficial in their recovery.
• Engage in active discharge planning to ensure smooth transitions for patients moving from the hospice setting to their home or other care environments, ensuring that ongoing support and resources are in place.

Our patient population encompasses individuals facing serious health conditions, including terminal illnesses, chronic disabilities, and significant life transitions. As a Senior LCSW, you will play an instrumental role in understanding and addressing the unique psychosocial needs of these patients, contributing directly to their emotional resilience and overall quality of life.

• A Master's degree in Social Work from an accredited program.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong communication and interpersonal skills, along with a demonstrated ability to build trusting relationships with patients and families.
• A thorough understanding of the psychosocial aspects of health, particularly regarding grief, adjustment disorders, and coping mechanisms.

• Previous experience in a rehabilitation or medical setting is highly desirable, enhancing your understanding of the multifaceted challenges faced by patients.
• Experience working with diverse populations and in interdisciplinary teams to promote holistic healing has a significant advantage.
