Our home health census is growing, and we're looking for compassionate and talented Speech-Language Pathologists (SLPs) to join our expanding therapy team in St. Clair Shores, Eastpointe, Grosse Pointe, Monroe, and Belleville!

What You'll Do
As a member of our home health therapy team, you'll provide personalized, one-on-one care to help patients improve communication, cognition, swallowing, safety, and overall quality of life in the comfort of their homes.
SLPs will:
- Complete comprehensive speech-language evaluations to assess communication, cognition, voice, and swallowing needs.
- Develop individualized plans of care and measurable treatment goals.
- Provide evidence-based interventions for speech, language, cognitive-communication, and swallowing disorders.
- Assess and treat conditions including dysphagia, aphasia, dysarthria, cognitive impairment, and other communication disorders.
- Reassess patient progress and modify treatment plans as clinically appropriate.
- Provide skilled speech therapy services in accordance with physician orders and CMS Conditions of Participation (CoPs).
- Educate patients and caregivers on communication strategies, swallowing precautions, aspiration prevention, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs).
- Maintain timely, accurate documentation in accordance with CMS, payer, and agency requirements.
- Collaborate with physicians, caregivers, nursing, therapy, and the interdisciplinary team to support successful patient outcomes.
Requirements
Our Ideal HHC Guru
You may be our next Home Health Magic Maker if you have:
- Degree from an accredited Speech-Language Pathology program.
- Current Michigan Speech-Language Pathologist license in good standing.
- Certificate of Clinical Competence (CCC-SLP) preferred, as applicable.
- Home health experience preferred.
- Strong clinical judgment, critical-thinking, and problem-solving skills.
- Excellent communication, interpersonal, and patient education skills.
- Ability to work independently while remaining connected to the interdisciplinary care team.
- Reliable transportation, valid driver's license, and current auto insurance.
- Familiarity with Medicare/CMS documentation preferred.
