Step-Down Navigator

The Vines Hospital (affiliated with Universal Health Services), located in Ocala, FL is a premier behavioral health facility offering individualized mental health and addiction treatment to adults and adolescents in a secure and private setting. Our services include partial hospitalization and intensive outpatient programs. We offer acute psychiatric, detox, and PTSD treatment. We are located in picturesque Marion County, FL. The county is known as the Horse Capital of the World. Enjoy the outdoors at one of the county's serene parks, which includes wonderful hiking trails, river rafting, and tubing. Considered the heart of Florida, the beautiful region is centrally located within hours of many activities. Just a drive away from some of the state's amusement parks or beautiful Florida beaches. Position Overview

Step-Down Navigator

The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. He/ She will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. Responsibilities include, but not limited to:

  • Meeting with eligible patients to discuss the benefits of step-down services as part of their continuum of care treatment.
  • Communicating eligibility to senior leadership and associated departments.
  • Collecting and entering data specific to eligibility into Midas for trending report purposes.
  • Coordinating between departments to ensure a smooth transition for patients.

Qualifications

  • Education: High school graduation or GED required. Bachelor's degree in social work or a clinical related mental health field, preferred.
  • Experience: Minimum two (2) years' experience in an inpatient psychiatric/mental health setting.
  • Licensure: Current CPR, Physical and Verbal Crisis Intervention Certification. Training may be obtained during new hire orientation.
