Job description

Responsibilities

The Clinic RN (Registered Nurse) provides skilled nursing care in accordance with InnovAge policies and all applicable regulations to participants.

  • Assists in the examination, testing, and treatment of participants.
  • Implements orders written by primary care providers.
  • Triages participant and staff phone calls to determine the priority of care and routes accordingly.
  • Schedules outside appointments for participants when necessary.
  • Administers prescribed medications and treatments. Provides training to participants in self-administration of applicable medications and monitors medication compliance.
  • Documents all necessary information and maintains participant medical record(s).
  • Provides health counseling and education to participants and their families.
  • Observes, records, and reports participant’s condition and reaction to drugs and treatments to providers.
  • Assist with clinical teaching and clinical supervision of CNA Day Center staff.
  • Develops, implements, and maintains a current nursing care plan.
  • Supervises LPN/LVN’s and Medical Assistants in addition to evaluating the overall effectiveness of the clinic in the absence of the Nursing Services Director/Clinic Nurse Supervisor.
  • Ensure that exam rooms and/or triage area is stocked for necessary supplies and materials and that appropriate staff maintains a list of medical supplies.
  • Effectively communicates and actively engages in interdisciplinary team meetings, family meetings, and clinic meetings.
  • Participates within the interdisciplinary team in the formulation of Plans of Care participants, as well as in other interdisciplinary team settings.
  • May be required to provide after hours and holiday coverage.
  • Complies with all regulatory and policy and procedure guidelines.

REQUIRED

  • Current State issued Registered Nurse License
  • Current First Aid and BLS certifications are required prior to hire. Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross.
  • 1+ yr working with the frail or elderly

PREFERRED

  • Bachelor’s degree in nursing
  • 1+ yr acute care nursing
  • 3+ yrs health care experience with emphasis in geriatrics
  • Bi-lingual-Spanish, Russian, Punjabi, or Hmong
