Job Description Job Description AME Home Care, LLC is a home care company based in Catonsville, Maryland. We are ateam of professionals who work well together and maintain a strong focus on the well-being of our clients. We are looking for an excited team player who can effectively multitask in a fast-paced, ever-changing environment.

You must be a fast learner and very detail-oriented. Responsibilities include but are not limited to: Perform duties related to the supervision, instruction, and motivation of home care aides. Filling all open shifts with aides based on the needs of the client, employee qualifications, and plan of care.

Ability to effectively build one-on-one relationships with both clients and caregivers. Timely and consistently document all information in the contact management system. Maintain a current roster of staff, their qualifications/licensures, and their availability.

Perform support duties for the nurses in-house and in the field. Monitoring telephony system to ensure caregivers are arriving to clients' homes as scheduled. Entering missing times/shifts in a timely fashion.

Participate in in-service education, quality assessment activities, and other staff meetings as required. Relay pertinent information about patients to the assigned staff, RN and Manager. Maintain an exceptional level of professionalism and responsiveness with prospective clients and others who may communicate with the agency.

Receiving and reviewing caregiver care notes. Minimum job requirements: High school diploma Minimum of 1 year experience as a care coordinator in personal care, home health care or related field. Excellent professional interpersonal and communication skills - oral, conversational, telephone and written.

Effective organizational and task management skills. Ability to demonstrate good judgment. Must work well in fast-paced environment and adapt quickly to changing conditions.

Must be proactive and able to successfully and effectively adhere to tight deadlines.
