This position is uniquely designed for remote engagement, allowing flexibility in work hours based on appointment schedules. As a member of a multidisciplinary team, you will collaborate with primary care providers to deliver integrated healthcare solutions, combining behavioral health services within a primary care setting. • Assist the primary care provider in identifying, assessing, treating, and managing mental health and psychosocial issues, acting as a proactive liaison within the primary care team.
• Conduct thorough patient intakes including diagnostic interviews and functional assessments, making informed recommendations to the primary care team regarding patients’ treatment goals.
• Deliver brief yet effective therapeutic interventions tailored to each patient’s unique mental health needs.
• Advise primary care providers on patients’ suitability for continued care in a primary care environment versus referral to specialized mental health facilities.
• Perform follow-up assessments to monitor patient progress and adapt treatment plans as necessary.
• Develop and implement relapse prevention plans, aiding patients in maintaining stable emotional and psychological functioning.
• Identify “at risk” patients and devise actionable plans to prevent deterioration of their mental health status.
• Coordinate health services across various levels of care, ensuring efficient communication between treatment providers with patient consent.
• Document patient progress meticulously in treatment charts while maintaining confidentiality.
• Maintain communication with primary care providers regarding patient needs and treatment efficacy, collaborating on innovative treatment strategies.
• Educate patients on self-management skills, empowering them to be active participants in their treatment.
• Provide insights and recommendations to clinic management concerning behavioral health service outcomes and program evaluations.
• Assist patients in adhering to medical treatments prescribed by primary care providers, including coping mechanisms for potential medication side effects.
• Uphold the Code of Ethical Conduct for Clinical Professionals while fulfilling additional duties as assigned.

• A Master’s Degree in Clinical Counseling or a closely related field is required.

• Familiarity with the DSM-IV diagnostic criteria and various therapeutic modalities, including family and group therapy.
• Strong interpersonal skills, enabling effective communication with patients, families, and healthcare teams.
• Mental and physical capability to handle the demands of the role effectively.
• Thorough understanding of the psychosocial impact that mental health struggles can have on patients and their families.
• Knowledge of community resources and their effective utilization.

• Experience working within a healthcare team, especially in a primary care or community health clinic setting.
• Proficiency in bi-lingual English-Spanish communication is preferred, to better serve our diverse patient base.
