Job description

When applying please be sure to indicate who you are going to provide alternate care for. We currently do not have any openings for generic alternate care, only for alternate care for a specific EFH.


An Alternate Care Provider provides back up care to an Extended Family Home (EFH) provider. An Extended Family Home Provider is a safe, loving and structured living environment with a family, couple or individual. Extended Family Homes provide adults with developmental and/or intellectual disabilities social and independent living skills training in a family setting.


This position is a work from the EFH's home position where you provide care and support for a person. Like a foster care situation but geared towards the social growth and wellbeing of an adult with disabilities in a stable home environment.


Prime Home DDS coordinates adults with developmental disabilities and the Extended Family Home. This position requires a desire to make a difference in an individual's life and commitment to the lengthy application process.


The applicant process is thorough and detailed, so it depends on many factors:


Accuracy/Completeness of Application.


Coordination of phone and face to face interviews.


No criminal background, APS/CPS and Sex Offender history.


Completion of required training.


The selection process is a judicious process and can be terminated at any time based on not meeting criteria set forth by the State and Prime Home DDS.
