Job description

O'Neill Healthcare Hospice is looking for a Liaison to join our team!

Duties and Responsibilities:

  • Visit patients in hospitals or in their homes to discuss benefits, financial implications, and processes of starting hospice and palliative care
  • Managing incoming referrals via fax or the electronic referral system (ECIN/Allscripts)
  • Develop professional relationships with case managers and discharge planners, hospital employees and management, patients, and families
  • Assist in strategic planning to identify referral sources both in the community and hospitals
  • Processes all new referrals for hospice care, both from internal and external sources
  • Educates potential customers of benefits and appropriateness of hospice care
  • Facilitate hospice consent meetings with patients and families
  • Completing clinical reviews via Allscripts and wellsky and/or onsite evaluation as needed and coordinating a seamless transition to assigned centers
  • Ensure insurance has been verified and complete authorizations as needed
  • Ensure all documentation is current in electronic record
  • Participate marketing events as needed


Requirements:

  • Experience in Healthcare marketing, hospice preferred.
  • Ability to use a computer and necessary software (ECIN/Allscripts, Wellysky and PCC experience preferred).
  • On call availability for occasional work outside of business hours.
