Seva Home Health is looking for an LVN for our growing Home Health business serving Tulare County and surrounding communities. Mileage is reimbursed and driving time is paid.

Requirements:

  • 2 years LVN experience, home health preferred
  • LVN license, required
  • IV Therapy/Blood Draw certification required
  • Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
  • Experience with Kinnser (WellSky) EMR preferred
  • Experience with Allscripts preferred
  • Reliable transportation required

Primary responsibilities will be:

  • Responsible for the oversight and delivery of quality, patient-centered care rendered in the patientโ€™s residence.
  • Follow the patient Plan of Care (POC) as directed by RN Case Manager and PCP
  • Communicate necessary revisions to patient status and/or needs to RN Case Manager and Clinical Manager when applicable
  • Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.
  • Provide complete, accurate and timely documentation consistent with the plan of care
