This role is heavily phone-based and requires strong clinical judgment, documentation accuracy, and comfort working in web-based systems.

What You'll Do

Conductoutbound calls(target volume:~100 calls/day) to complete CMRs and MTM services Provide patient consultation and education, including statin-related guidance aligned withAHA guidelines Accurately document/transcribe patient-specific: prescription details diagnoses recommendations into athird-party web-based application Contribute to standardized documentation that will be mailed to patients Attend trainings to build/refresh role-specific skills Required Qualifications Active, valid Pharmacist license in Florida (FL) Please include a screenshot of your FL license at the top of your resume 4+ yearsexperience as aclinical MTM pharmacist(management experience preferred) Experience withMedicare Part D Star measures, especiallySUPD, SPC, and CMR Strong knowledge of medical/pharmacy terminology and medication review best practices Strong English communication skills (verbal + written) Tech-savvy; comfortable moving quickly in online tools/applications Ability to maintain confidentiality; detail-oriented; organized; sense of urgency Preferred Salesforceexperience
