RN Case Manager
Full-Time

The RN Case Manager is responsible for providing skilled nursing care and coordinating services for patients receiving home health care services. The RN Case Manager works closely with physicians, patients, caregivers, and the interdisciplinary team to ensure quality care and positive patient outcomes.
Essential Duties & Responsibilities
  • Perform patient assessments and develop individualized care plans
  • Complete admissions, routine visits, recertifications, and discharge visits
  • Administer skilled nursing care according to physician orders
  • Educate patients and families regarding disease management and treatment plans
  • Coordinate care with physicians and interdisciplinary team members
  • Ensure timely and accurate clinical documentation
  • Monitor patient progress and communicate changes appropriately
  • Participate in care conferences and quality improvement activities
  • Maintain compliance with agency policies and regulatory requirements

Qualifications
  • Current RN license in applicable state
  • Home Health experience preferred
  • Strong clinical assessment and documentation skills
  • Excellent communication and time management skills
  • Current CPR certification
  • Valid driver's license and reliable transportation
