Job description

Job Summary:
  • Experience Level: Mid-Senior (5+ to 7 years total; 6 years Case Management; 2 years supervisory/management).
  • Education: Master's degree in business, social work, nursing, or healthcare-related field (minimum Bachelor's noted).
Key Responsibilities:
  • Manage daily operations and staff in Case Management Department including care coordination, discharge planning, utilization management, social work, and pre-admission services.
  • ct as liaison between patients, families, staff, physicians, healthcare team, community agencies, managed care, and skilled nursing facilities.
  • Establish, implement, and monitor best practices and clinical standards for quality patient care and organizational financial integrity.
Required Qualifications:
  • Master's degree from accredited school in business, social work, nursing, or healthcare-related field.
  • Washington Registered Nurse License upon hire.
  • 6 years experience in Case Management.
  • 2 years supervisory/management experience, preferably in Case Management.
  • Previous nursing experience in complex client care areas.
  • Experience in problem-solving, conflict resolution, and adult education for multi-disciplinary staff.
  • Preferred Qualifications:
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) upon hire.
  • Case Management and supervisory experience in acute care hospital.
