Centra's PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that's most appropriate for each participant-whether that's in their own home, with family, in long-term care, or assisted living.
 

PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated with a dedicated interdisciplinary team of providers, nurses, dietitians, physical and occupational therapists, social workers, activities coordinators, and transport staff.

Centra Medical Group (CMG) is a comprehensive network of nearly 600 employed providers within Centra Health. Centra Health services a population of 600,000 community members with 70 different outpatient and specialty practices. Centra offers primary care and specialty services across Central and Southern Virginia-from Lynchburg to Danville, Farmville, Moneta, and Bedford. CMG includes family practices, specialists, and surgeons in fields such as cardiology, neurosurgery, psychiatry, urology, and more.

  • MD/DO-Experience with older adults required

  • Board Certified or Board Eligible in medical specialty (Family Med, Palliative/Hospice/IM, Em Med)

  • Experienced providers in palliative, hospice, primary care, or long-term care are encouraged to apply

  • HCC coding experience/familiarity valued, but not required

  • Collaboration with Multidisciplinary, collegial team: Physician (or APP), PT/OT, MSW, Dietician, Nurse, Transport, Activities Coordinator, Site Director, Home Care

  • Team Based Care-IDT meetings in afternoon 2x weekly

  • Facility includes craft room, memory care, therapy gym, garden, day room, office space

  • Schedule-M-F, most participants have left the facility by 3:30pm, nursing visits (ALF, LTC, SNF) as needed and occasional home visits

  • Extended 1:1 time with patients, avg 3-9 per day, additional time for care coordination and IDT

  • Vacation: 5.5 weeks 

  • Call is equally shared by all providers, currently every 6 weeks. Nurses take all calls and escalate to the provider as needed. Phone call only.

  • Cerner EMR 

  • Hospital Employed 

  • Teaching opportunities available

  • Provide acute and episodic care visits, including required regulatory assessments.

  • Travel as needed to hospitals and partner facilities, including Assisted Living Facilities (ALFs), Skilled Nursing Facilities (SNFs), patient homes, and hospitals.
