Job description

POSITION TYPE: ON-SITE
SALARY RANGE: $48,000.00-$74, 080.00
POSITION SUMMARY
The Assertive Community Treatment (ACT) Rehabilitation Specialist II (RS II) functions as part of a 10-12-person clinical team to deliver ACT Services to a group of consumers receiving services at the Community Connections Core Service Agency. Each RS II has responsibility, under the supervision of a licensed Clinical Manager (Supervisor), for working directly with a caseload of 10-12 individuals while also participating in case sharing with all consumers assigned to the team.
KEY RESPONSIBILITIES
  • Participate as team member in the development of a consumer's Individual Recovery Plan (IRP) under the direction of the Clinical Supervisor.
  • Assume responsibility for the implementation of the IRP in coordination with the team and under the supervision of the team Supervisor.
  • Develop therapeutic relationship with assigned clients to enhance functioning.
  • Intervene to assist consumers in acquiring skills and resources necessary for successful community living. In so doing, the RS should spend at least 65% of their time in direct contact with consumers and network members either face-to-face or via telephone.
  • Provide support and consultation to members of the consumers' network. In so doing, the RS should spend at least 75% of his/her time in the field including home visits.
  • Assist the consumer in symptom management, problem solving, and resource development.
  • Participate in crisis prevention and intervention on behalf of the team's clients as needed.
  • Ensure that each consumer has an individualized relapse prevention plan that is shared with all appropriate providers and network members.
  • Participate in leadership of psycho-educational and skill development groups as appropriate.
  • Participate in ACT Team daily morning meetings.
  • Develop strategies with the consumer so that he or she can maintain living outside of institutional settings.
  • Complete diagnostic assessments and treatment plans under the supervision of the team Supervisor.
  • Assume responsibility for special projects on the team and within the ACT program.
  • Complete all required paperwork on time.

In addition to role responsibilities, each staff member of Community Connections has the following responsibilities as a part of their employment:
  • Models and reinforces Community Connections mission to provide behavioral health, residential services, and primary health care coordination for marginalized and
    disenfranchised women, men, youth, and children living in the District of Columbia, many of whom are coping with challenges including mental illness, addiction, and the aftermath of trauma and abuse.
  • Models and reinforces Community Connections values of quality, innovation, respect, equity, and integrity daily.
  • Reinforces Community Connection's commitment to diversity, equity, and inclusion.
  • Protects the privacy of our consumer's protected health information by maintaining compliance with HIPAA and other relevant CC related IT security regulations.
  • Completes and stays current on role specific and organizational wide training.
  • Performs other duties as assigned on an as-needed basis.

DESIRED KNOWLEDGE/SKILLS/ABILITIES
  • Masters' degree in social work, psychology or relevant human services discipline.
  • Active LGSW or LCPC license issued in the District of Columbia.
  • Demonstrates knowledge of mental health principles and demonstrated ability to operationalize and implement these principles in mental health practice.
  • Proficient in Microsoft Office products (Word, Excel, Outlook).
  • Case Management experience preferred.
  • Experience documenting case notes preferred.
  • Ability to identify and resolve problems in a timely manner even when dealing with emotional topics.
  • Solves practical problems and manages a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions in both written, oral, diagram, or schedule form.
  • Ability to understand complex clinical concepts (engagement, outreach, empathy, etc.) and interventions over time and to incorporate these into clinical practice under supervision of Clinical Manager.

POSITION TYPE/EXPECTED HOURS OF WORK
  • May need to work untraditional work hours such as weekends, evenings, observed holidays, and nights.
  • This is a position which may report to work under an office occasional model as defined by the business needs of Community Connections, Inc.
