About the Role

We are looking for a dedicated Registered Nurse (RN) – After Hours to join our hospice team full-time.

In this role, you’ll provide evening and overnight support for patients and families, respond to urgent needs, make clinical assessments, and coordinate care with the interdisciplinary team. You’ll be the reassuring voice and presence families rely on during critical moments. This position travels throughout the whole Blue Ridge Care territory

How You’ll Make a Difference

• Answer emergent calls from patients and families.

• Provide after-hours visits to address immediate concerns or changes in condition.

• Deliver skilled nursing care based on patient goals and the physician’s plan of care.

• Assess and evaluate patient status; give guidance and oversight when LPN support is involved.

• Develop and update care plans based on nursing assessments.

• Administer medications and treatments as prescribed.

• Support preventive, restorative, and rehabilitative care needs.

• Educate patients and families on symptom management and care techniques.

• Offer support and reassurance during challenging and emotional moments.

• Complete timely, accurate clinical documentation.

• Notify physicians of changes in condition and obtain/document orders.

• Ensure the home environment is safe and appropriate for care.

• Coordinate needed equipment and services.

• Conduct initial assessments and support smooth transitions across the care team.

• Collaborate with families and the interdisciplinary team to align care with the patient’s goals.

• Participate in continuing education and quality activities.

• Support programs such as peer chart reviews, medication reconciliation, and tuck-in calls.

 What You’ll Bring

• Current RN license in Virginia

• Graduate of an accredited nursing program; BSN preferred

• Two or more years of hospice, home health, or ICU/CCU experience preferred

• CHPN certification preferred

• Current CPR certification

• Strong communication and interpersonal skills

• Knowledge of Medicare/Medicaid hospice regulations

• Proficiency with electronic medical records

Ability to travel throughout the Blue Ridge Care territory

• A calm, confident presence during urgent and emotional situations
