About the Position
We are seeking a caring and detail-oriented Registered Nurse to join our AdultFoster Care (AFC) program as an Initial Assessment / Admission Nurse. In thisrole, you will complete in-home evaluations for individuals applying to the AFCprogram, reviewing their needs and confirming eligibility. This opportunity isideal for nurses with experience in geriatrics, home health, or community-basedcare who are committed to supporting individuals and enhancing quality of life.


Key Duties

  • Conduct comprehensive in-home assessments for new AFC applicants.
  • Evaluate medical, functional, and psychosocial conditions to determine eligibility.
  • Partner with caregivers, families, and healthcare professionals to design customized care plans.
  • Provide education to participants and families regarding AFC services, expectations, and program policies.
  • Ensure compliance with all state regulations and AFC program requirements.
  • Maintain accurate, up-to-date documentation of assessments, care plans, and follow-up activities.


Qualification
