Brockton Home Health Care AFC is looking for A registeredNurse (RN)/Case Manager to manage and support our Adult Family/Foster to makesure the our programs runs smoothly.
Jobdescription
โ€ข Responsible for overall supervision of all HomeVisits RN's;
โ€ข Completing the MDS-HC and other clinicalassessments;
โ€ข Developing and reviewing on an ongoing basiseach member's AFC/GAFC plan of care;
โ€ข Training, evaluating, and supervising AFC/GAFCcaregivers in conjunction with Program Director;
โ€ข Reviewing the physician summary;
โ€ข Monitoring the health status of all members toensure that all needed AFC/GAFC is properly delivered;
โ€ข Reporting changes in the health status of anymember to the member's physician;
โ€ข Periodically reviewing AFC/GAFC caregiver logs;
โ€ข Conducting admission on-site visits for newmember;
โ€ข Completing a nursing progress notecorresponding with each on-site visit or encounter;
โ€ข Submitting a semiannual health-status report tothe member's primary care physician;
โ€ข Planning for an implementing discharges fromthe AFC/GAFC program;
โ€ข Conducting an orientation for each AFC/GAFCcaregiver before the AFC caregiver begins personal care;
โ€ข Providing ongoing training to AFC/GAFCcaregivers on health and aging
โ€ข Any other office duties that may be assigned byProgram Directors.
Qualification
โ€ข Demonstratedability to coach, train and provide staff development
โ€ข Proficiencyin Microsoft (Word, Excel and PowerPoint)
โ€ข MS or Bachelor'sdegree in Nursing
โ€ข Must befully licensed RN by Mass Board of Registration
โ€ข At leastfive years of previous home health care experience
