This position is ideal for a nurse who enjoys building relationships with clients, completing assessments, and working closely with caregivers and families to deliver exceptional care.

Responsibilities
  • Complete Start of Care assessments for home health and private duty clients
  • Perform nursing assessments and supervisory visits
  • Develop and update individualized care plans
  • Coordinate care with physicians, therapists, caregivers, and family members
  • Educate clients and families regarding disease management and safety
  • Ensure compliance with state regulations and agency policies
  • Maintain timely and accurate electronic documentation
  • Participate in quality assurance and clinical oversight activities
  • Assist with new client admissions as needed
Qualifications
  • Active Florida Registered Nurse (RN) license in good standing
  • Minimum of 2 years of nursing experience preferred
