Job description

Empath Home Health, a member of Empath Health, is seeking a caring and motivate Home Health Baylor Registered Nurse to make home visits in Marion, Lake, and Sumter counties. The Baylor RN will work Friday-Sunday (3 days per week) OR Friday-Monday (4 days per week).

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.
Locations: Home health visits in the Ocala area (Marion, Lake, & Sumter counties)
Schedule: This is a Weekend Baylor role requiring full weekends. You decide which schedule below fits your life!
  • 4 days (Friday-Monday, Thursday-Sunday, OR Saturday-Tuesday)
  • 3 days (Friday-Sunday OR Saturday-Monday)

What you'll Do
  • Performs initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
  • Initiates the plan of care and revise as necessary.
  • Provides nursing services and/or treatments indicated on plan of care.
  • Counsels and educates the patient and family on disease process, self-care techniques and prevention strategies.

What You'll Need
  • Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
  • BSN or MSN preferred.
  • Minimum one year experience providing direct patient care; previous experience as a home health RN preferred.
