Key Responsibilities - Behavioral Health RN (Home Health)
  • Perform in-home behavioral health and psychiatric nursing assessments, including mental status and safety evaluations
  • Deliver skilled home health nursing interventions, including medication management and symptom monitoring
  • Provide patient and caregiver education on mental health conditions, treatment adherence, and coping strategies
  • Develop and implement individualized behavioral health care plans in a home and community care setting
  • Coordinate care with physicians, therapists, and interdisciplinary teams
  • Maintain accurate, timely documentation in accordance with home health standards

Qualifications - Behavioral Health Nurse (RN)
  • Active California Registered Nurse (RN) license in good standing
  • RN-BC certification is requiredΒ 
  • Minimum 1 year of behavioral health or psychiatric nursing experience (home health or community care preferred)
  • Graduate of an accredited nursing program (BSN preferred)
  • Strong skills in clinical assessment, crisis intervention, and therapeutic communication
  • Public Health Nurse (PHN) certification (preferred)
  • Valid driver's license and ability to travel throughout Ventura County
