At WTCSB our mission is to provide a coordinated system of quality recovery-oriented care in Mental Health, Intellectual Disability, and Substance Abuse Services to the citizens of the cities of Franklin and Suffolk and the counties of Isle of Wight and Southampton. We are seeking qualified Case Managers for various departments throughout our organization

**ID/DD experience is preferred for this position. **

Responsibilities and Duties for ID/DD Case Manager:

    • Responsible for consumer assessment, service linkage, coordination, development, and implementation of a case management ISP
    • Assist and provide follow up instruction, education, and counseling to consumers/families with ID/DD
    • Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits
    • Manage consumer records according to all Licensure and Medicaid standards.

Qualifications and Skills forID/DDCase Manager:

    • BA/BS from an accredited college or university with major course experience in Psychology, Social Work, or related field
    • One year of clinical experience in a Human Services field
    • QMHP or QIDPpreferred
    • KSA's (Required)
    • Valid Driver's License with an acceptable DMV record at the time of employment and continuously thereafter
