Job description

The Advanced Practice Registered Nurse or Physician Assistant, referred to as APP provides comprehensive preventive and primary care services consistent with PCMH principles, as part of the clinical provider team, to patients of the Bluegrass Community Health Center.

Provides direct preventive and primary care for pediatric, adults and geriatric patients. This includes providing care related to health maintenance, acute care, and chronic disease management at the health center or in outreach settings in a manner consistent with Patient Centered Medical Home principles.

  • Provides direct medical care for each patient, including:
    • Complete medical history
    • Physical examination
    • Recommended screenings
    • Diagnosis (es)
    • Plan of care including appropriate treatment and/ or referral
    • Follow-up/ongoing care
  • Provides high-quality, culturally-competent, linguistically appropriate care
  • Thoroughly and correctly documents medical encounters in health center’s electronic medical records system.
  • Correctly codes medical encounters according to level of patient encounter.
  • Orders and uses all available resources in diagnosis and treatment: i.e. labs, x-ray, in an appropriate and cost efficient manner.
  • Confers with other clinical team staff members, outside experts when necessary, and with patient family, as appropriate, concerning treatment and care of patients.
  • Refers patients in need of additional diagnostics, specialty care or support services to appropriate referral source, reviews referral results in a timely manner and communicates result to patient.
  • Provides health education and demonstrates effective communication.
  • Achieves BCHC provider team productivity goals and quality measures:
    • Works efficiently and effectively to meet productivity benchmarks and quality measures per practice agreement.
    • Provides leadership to medical team through effective communication, teaching and negotiation
  • Will share call, one week at a time, equally.

Acts as leader of a Patient Centered Medical Home healthcare team

  • Leads PCMH daily planning huddles.
  • Provides guidance to licensed and unlicensed team members.
  • Participates and assists in staff development/mentoring activities.
  • Exerts final decision in clinical matters in accordance with health center policies and procedures.

Actively participates in administrative activities necessary for operation of a Patient Centered Medical Home community health center.

  • Maintains and continually updates clinical knowledge.
  • Attends meeting and/ or seminars as requested by Medical or Executive Directors.
  • Provides excellent customer service.
  • Embraces the mission of the organization.
  • Provides clinical instruction to staff or community partners as requested, or required.
  • Participates in, or possibly directs, quality assurance/performance improvement initiatives.
  • Demonstrates evidence of continuing professional growth by involvement in peer review and the review of protocols and procedures; participation in or designing of continuing education
  • Participates in other activities as requested by Medical or Executive Director.

    Minimum Education Required:

  • Graduation from an accredited APRN/FNP or accredited Physician Assistant program.
  • The candidate is required to maintain continuing medical education credits in quantity sufficient to satisfy requirements for re-licensure.
  • APRN: two years of experience is required.
  • PA: 3 years of experience is required.
  • Preferred:

  • Spanish language fluency
  • 3 years of work experience
  • Previous work in community health centers
  • License: Must hold RN license and APRN certification or PA License in the state of Kentucky.

    A criminal background check is required.
