Job description

PRN Hospice Advanced Practice Provider

Join our growing, well-respected team with the full support of St. Elizabeth Healthcare, an award-winning hospital system. We are seeking a PRN Hospice Advanced Practice Provider to contribute to our mission of delivering compassionate, patient-centered care.

Position Highlights

Schedule: 12-16 hours per weekend.

Location: Edgewood, rounding on the inpatient unit and taking calls during an 8-hour day.

Responsibilities

Provide exceptional hospice care, including face-to-face visits for eligibility assessments, symptom management, and consultation with the hospice interdisciplinary team and medical director.

Key Responsibilities

Complete hospice face-to-face visits before the third and subsequent hospice benefit periods to assess eligibility based on the hospice conditions of participation and local coverage determinations.

Collaborate with the hospice medical director to provide clinical information for recertification decisions.

Coordinate and oversee patient care in consultation with the interdisciplinary team and other healthcare practitioners.

Ensure appropriate symptom management through interdisciplinary collaboration.

Develop and review treatment protocols, proposing interventions based on evidence-based literature.

Consult with the collaborating physician for plans of care outside standardized practice.

Provide expert on-call hospice care, addressing patient needs as they arise.

Required:

Current state licensure in KY for scope of practice.

Minimum of one (1) year of experience as a Nurse Practitioner.

Minimum of one (1) year of hospice or palliative nursing experience.

Strong clinical knowledge in areas such as advanced anatomy, pharmacology, pathophysiology, and clinical decision-making.

Excellent observation, communication, and problem-solving skills.

Familiarity with community resources and health promotion principles.
