Job description

Job Title: Ambulatory/Primary Care (Outpatient CBOC) RN

Location: Miami VA Healthcare System (including 7 Community-Based Outpatient Clinics) 

Period of Performance: 03/12/2025 – 03/11/2030

Shift Schedule: 8:00am-4:30pm 

Position Description: The Primary Care Registered Nurse (RN) delivers high-quality, patient-centered care across diverse clinical settings, including Primary Care/Patient Aligned Care Teams (PACT), Medical Specialty Clinics, Cardiac Catheterization/Electrophysiology Laboratory (CCL), and Gastroenterology (GI) services. This role supports Veterans in a multidisciplinary outpatient environment by combining clinical expertise, care coordination, patient education, and procedural assistance.


Skills: The Primary Care/ Patient Aligned Care Team (PACT)/Primary Care RN will work as the care coordinator for the PACT. This is an outpatient position. RN Shall will be responsible to check-in patients, provide disease management, scrub appointments, increase patient access through MyHealthyVet/secure message responses, collaborate with community care, and all duties as assigned. The outpatient RN shall be responsible for patient assessments, medication administration, IV therapy, interdisciplinary team coordination, telemetry monitoring, admissions, and discharges, internal and external transfers, delegating to assistive personnel, electronic medical records documentation, patient transport, and emergency response procedures with at least two (2) years nursing experience in an outpatient setting.


Requirements: 

Primary Care RN shall have BLS certification, and at least two (2) years of inpatient experience in care coordination. Medical Specialty RN shall have ACLS and BLS, three (3) years experience in an Outpatient setting (i.e.), Telemetry and Acute Care experience. The personnel assigned by the Contractor to perform the services covered by this task order shall possess an unrestricted license to practice medicine in one of the States, Territories, or Commonwealths of the United States or the District of Columbia.
