We are seeking a compassionate and skilled Occupational Therapist (OT) to provide individualized therapy services to our pediatric and adult residents. Position Overview

The Occupational Therapist is responsible for evaluating, developing, and implementing individualized treatment programs that promote independence, improve functional performance, and enhance quality of life for pediatric and adult residents.

The OT works collaboratively with physicians, nurses, respiratory therapists, physical therapists, speech-language pathologists, caregivers, families, and other interdisciplinary team members to improve residents' ability to safely perform activities of daily living, develop fine motor and cognitive skills, address sensory processing needs, and participate more independently in everyday routines.

Reports to: Director of Nursing (DON)

What You’ll DoOccupational Therapy Evaluation & Treatment

• Perform comprehensive occupational therapy evaluations for pediatric and adult residents
• Assess activities of daily living (ADLs), instrumental activities of daily living (IADLs), fine motor function, sensory processing, cognition, visual perception, and functional mobility
• Develop individualized treatment plans based on physician referrals, clinical findings, and resident-centered goals
• Implement evidence-based therapeutic interventions designed to improve functional independence
• Monitor resident progress and modify treatment plans as clinically appropriate

Pediatric & Developmental Therapy

• Provide age-appropriate occupational therapy interventions for pediatric residents
• Support developmental milestones and functional skill development
• Address sensory processing needs through individualized therapeutic techniques when appropriate
• Develop fine motor skills, coordination, self-care abilities, and functional participation
• Promote participation in play, school readiness, self-care, and everyday routines
• Collaborate with families and caregivers to encourage carryover of therapeutic activities within the home environment

Activities of Daily Living & Independence

• Provide therapeutic interventions to improve dressing, grooming, bathing, feeding, toileting, and personal hygiene skills
• Improve upper-extremity strength, coordination, range of motion, and fine motor function
• Develop interventions that maximize resident independence and participation in daily routines
• Evaluate and recommend adaptive equipment, splints, and assistive devices to improve safety and function
• Educate residents, families, and caregivers on adaptive techniques that promote independence

Ventilator, Tracheostomy & Medically Complex Care

• Collaborate with Respiratory Therapists and nursing staff when treating residents requiring ventilators, tracheostomies, oxygen therapy, and G-tube care
• Adapt therapeutic activities based on each resident's respiratory status, endurance, and medical complexity
• Recommend positioning strategies that improve comfort, functional performance, respiratory support, and pressure injury prevention
• Monitor resident tolerance to therapy and communicate changes or concerns promptly to the interdisciplinary team

Interdisciplinary Care Coordination

• Collaborate with physicians, nurses, respiratory therapists, physical therapists, speech-language pathologists, case managers, caregivers, and families
• Participate in interdisciplinary care conferences and treatment planning
• Communicate resident progress, barriers, and recommendations to the care team
• Educate caregivers and direct care staff on adaptive techniques, safe positioning, and therapeutic activities that promote functional independence

Resident & Family Education

• Educate residents and families regarding treatment plans, adaptive equipment, therapeutic techniques, and home exercise programs
• Provide practical strategies families and caregivers can incorporate into daily routines
• Encourage resident participation in therapy goals and promote the highest achievable level of independence
• Provide compassionate support and education regarding long-term rehabilitation goals and progress

Documentation & Compliance

• Complete evaluations, treatment notes, progress reports, recertifications, and discharge documentation accurately and timely
• Ensure therapy services are consistent with authorized plans of care and applicable payer requirements
• Maintain documentation supporting medical necessity and services provided
• Maintain HIPAA compliance and resident confidentiality
• Participate in quality improvement initiatives and clinical audits
• Complete required organizational and regulatory training
• Follow applicable reporting requirements for incidents and suspected abuse, neglect, or exploitation

Qualifications

• Current Occupational Therapist (OT) license in the State of Florida
• Degree in Occupational Therapy from an ACOTE-accredited program as required for Florida licensure
• Current CPR/BLS certification
• Ability to successfully complete required Level II background screening and credentialing requirements
• Experience in home health, rehabilitation, long-term care, pediatric care, or medically complex populations preferred
• Pediatric Occupational Therapy experience preferred
• Experience working with individuals requiring ventilator, tracheostomy, oxygen, and/or G-tube support preferred
• Experience working with individuals with developmental or intellectual disabilities preferred
• Experience evaluating and recommending adaptive equipment and assistive devices preferred
• Strong clinical assessment and treatment planning skills
• Excellent communication and interdisciplinary collaboration abilities
• Strong documentation and organizational skills
• Compassionate, resident-centered approach to care
• Ability to adapt treatment plans to meet the unique needs of pediatric and adult residents
• Ability to travel to assigned client homes and group home locations
• Ability to lift or move 50 pounds or more, with or without reasonable accommodation
