Community Outreach & Enrollment Coordinator

Location: Bainbridge Adult Day Care, 3529 Wayne Ave

Job Summary:

Must have Social Work experience in LTC facility

Bainbridge Adult Day Care is seeking a motivated Community Outreach & Enrollment Coordinator to drive participant (patient/client) recruitment. This role focuses on building relationships with healthcare providers, community organizations, and families to increase enrollment and awareness of our adult day care services for seniors and adults with disabilities.

Key Responsibilities:
  • Develop and implement strategies to recruit new participants for adult day care services

  • Build and maintain referral relationships with hospitals, social workers, case managers, nursing homes, rehab centers, and home care agencies

  • Conduct outreach to community centers, senior centers, and local organizations

  • Educate families and caregivers about the benefits of adult day care services

  • Coordinate tours, intake meetings, and enrollment processes for prospective participants

  • Represent Bainbridge Adult Day Care at health fairs, community events, and networking opportunities

  • Track leads, referrals, and conversion rates

  • Assist families with application paperwork and eligibility requirements (e.g., Medicaid, insurance, or private pay)

  • Collaborate with internal staff to ensure smooth onboarding of new participants

Qualifications:
  • Experience in healthcare outreach, admissions, marketing, or community relations

  • Strong network in the Bronx/NYC healthcare and social services community (preferred)
