SUMMARY OF POSITION

Provides initial in-depth information for all potential consumers looking for information related to Older American Act (OAA) programs.  Under the direction of the Program Supervisor, provides training on basic processes and procedures to trainees and ongoing workers in the ADRC Unit.  Works with management team to identify training goals and needs, develop and deliver training in the areas of agency service practices and State Medicaid policy. Act as a back-up as needed for the ADRC call center.

Performs position duties in a manner which promotes customer service and harmonious working relationships, including treating all people courteously and respectfully.  Engage in effective team participation through willingness to assist and support co-workers, supervisors, and other work-related associations. Develops good working relationships with division and agency staff and supervisors through active participation in accomplishing group projects and in identifying and resolving problems in a constructive manner.  Demonstrates openness of constructive feedback and suggestions, in an effort to strengthen work performance.  Contributes to a positive, respectful and productive work atmosphere.  Regular attendance is required to meet the demands of this job and to provide necessary services.

ESSENTIAL JOB DUTIES

Essential Duty 1: Options Counseling: 40% 

• Works with individuals in their own home or over the phone, to explore and plan for current and future care needs. 

• Explores options with individuals of different types of care providers and settings available, so that the individual can make an informed decision.  

• Identify and provide consumers, families, and caregivers with information on resources and services available. 

• Works with the individual to create short- and long-term care related goals. 

• Provides regular monthly monitoring with the individual while their action plan is open.

• Completes a follow up exit interview within 90 days.  

Essential Duty 2: Family Caregiver Support Program and relatives as Parents Program: 20% 

• Meets with family caregivers through home visits, phone calls, or virtual meetings to gather and complete information for the Oregon Caregiver Assessment Tool. Provides emotional support and may assist with resources location

• Coordinates services and resources by identifying barriers, problem-solving, and recommending resources that align with person-centered caregiving goals.
• Connects consumers with community services and benefits. 

• Provides regular check-in calls quarterly to inquire about the need for additional referrals or resources
• Authorizes stipend payments for those prioritized based on risk assessment scoring, and as budget allows, for respite care or supplemental supportive services.

• Conduct outreach and marketing efforts to engage with target populations and increase awareness of program and needs of family caregivers. 

Essential Duty 3: Provide training and technical assistance within the ADRC unit 20%

• Orient new employees within the ADRC unit classification
• Provide ongoing staff technical assistance with unit-related questions.              
• Assists with trouble shooting data entry systems.     
• Demonstrates understanding of all job duty aspects of the ADRC Specialist
• Create and maintain training materials used by the ADRC.  
• Attends local unit and state meetings, unit huddles, staffing and trainings as directed by supervisor.

Essential Duty 4: Provides back-up support for ADRC 15%

• Provides information and referrals to programs administered by ORCW as well as other community programs and service providers.
• Provides first contacts with consumers through telephone calls, email, in their homes, or walk in contacts.
• Screens and assists individuals with identifying long term and short-term care needs, programs, and benefits available to consumer based on income and service criteria.
• Provides service referrals based on state managed ADRC database as directed in ADRC policy.
• Mails information about services and applications for services to consumers.
• Responds to situations requiring short-term assistance.
• Responds to referrals made on behalf of consumers by service providers and community members.
• Interviews consumers, service providers, and community members regarding consumer service needs.
• Arranges for and/or develops resources for providing assistance from family, neighbors, churches, service organizations, etc. as needed.
• Coordinates with medical professionals and mental health professionals as needed.
• Receives all Preadmission Screening (PAS) requests from hospitals and nursing facility staff and enters screening data for PAS case managers.

 

Outreach 5% 

• Conducts community outreach to increase awareness of ADRC helpline, with a focus on priority populations.

• Identify and share available resources, programs and services that can benefit tribal communities  
• Coordinates events to support family caregivers and facilitate evidence-based programs and education in the community.  

OTHER JOB DUTIES

• Acts as a mandated reporter of abuse and neglect of adults over age 65, adults with physical disabilities, adults with developmental disabilities, adults with a mental illness, and any children under 18 years, to appropriate agency.
• Performs other related duties as assigned by supervisor. 

WORKING ENVIRONMENT AND PHYSICAL DEMANDS

Work is generally performed in an office environment with routine computer and telephone usage. Position requires sitting and typing for long periods of time. Position may require attendance at occasional evening meetings. Frequent travel to consumers’ homes and other community locations throughout the three-county area may be required to perform assessments, pick up meeting supplies, run errands for the program, attend trainings, or visit other COG work sites.

SUPERVISION RECEIVED: Receives general supervision from the Program Supervisor.

SUPERVISORY RESPONSIBILITIES:  None.

MINIMUM QUALIFICATIONS

Experience & Training

Bachelor’s degree preferred, or associate degree plus three (3) years’ experience in social services, along with experience in customer relations, call management, information and assistance or related field that includes phone assessment and triage skills; OR any satisfactory equivalent combination of education, training and experience which demonstrates the knowledge, skills, and abilities to perform the duties of the job.

 Knowledge, Skills and Abilities

• Knowledge of and/or experience working with information and referral taxonomy;
• Broad knowledge of and familiarity with the issues and concerns of older adults and people with disabilities;
• Knowledge of, and ability to, operate a personal computer;
• Knowledge of, and experience in, Microsoft software;  
• Ability to acquire and apply confidentiality rules regarding client and provider records;
• Ability to pay attention to detail and maintain accurate, up-to-date records, and prepare reports using ADRC resources;
• Ability to summarize data, write accurate, concise, and clear narratives and reports;
• Ability to effectively interview and communicate with older adults and/or people with disabilities, and/or their family members;
• Ability to work independently, organize and structure work, handle multiple projects and a high volume of work;
• Ability to initiate own work with minimum supervision;
• Good interpersonal and customer service skills and the ability to effectively communicate verbally;
• Ability to establish and maintain positive and effective working relationships with staff, consumers, other agencies, and the public;
• Ability to prepare and deliver effective presentations;
• Ability to assess individuals in their own home to determine appropriate service and/or community referral.

Licenses, Certificates, & Other Requirements

Possession of a Valid Oregon Driver’s License, or the ability to obtain reliable transportation in order to travel throughout the area.

Ability to be certified through the Alliance of Information Referral Specialist (AIRS). Position requires successful completion of a criminal history and driving record check.
