Edenbrook Appleton, part of the Eden Senior Care Network, a skilled nursing facility is seeking for an Admissions Director for our wonderful skilled nursing facility to make a difference in the lives of our residents. 

Position: Admissions Director

Responsibilities:

  • Complete daily rounds in designated hospitals
  • Obtain new high acuity inquiries and assess special patients to evaluate care needs before admission, and directs to the appropriate center
  • Evaluate inquiries, make the decision, communicate information to the case manager and admissions coordinator, and facilitate the patient’s smooth transition into the center
  • Obtain accurate information from physicians, patients, and referral sources regarding the expected discharge plan, and communicate this information to the interdisciplinary team
  • Develop relationships with hospital social workers, physicians, hospital discharge planners and administrators, clergy, attorneys, civic/professional organizations, home health agencies, families, etc., to understand the need for program services and to communicate services offered which meet these needs

Requirements:

  • Knowledge of area hospitals and other local healthcare organizations is preferred, but willing to train.
  • Experience with physician and hospital staff relationship development
  • Sales and marketing experience in generating referral sources
  • Has established relationships with local social workers and discharge planners
  • Excellent presentation and relationship-building skills
