• Up to $295,000 (Experienced Physicians)
• Annual Incentive Bonus: $45,000 - $85,000 (Achievable Metrics)

Position Overview
The Primary Care Physician (PCP) provides comprehensive, high-quality care within a collaborative, team-based environment focused on value-based outcomes.
Physicians are supported by care teams and quality-based coders to ensure strong documentation, coordinated services, and efficient workflows - allowing providers to focus on meaningful patient interactions.
This is a structured model with minimal call requirements and clearly defined quality metrics tied to bonus compensation.
Key Responsibilities
• Deliver high-quality primary care aligned with clinical standards
• Coordinate comprehensive patient services, including:
- Specialist referrals
- Hospital and SNF coordination
- Durable medical equipment
- Home health services
• Participate in daily care team huddles
• Collaborate closely with Lead Physician and Center Administrator
• Complete daily medical record documentation
• Work alongside quality-based coders for accurate and comprehensive documentation
• Adhere to clinical leadership policies and protocols
• Review quality and outcome data regularly with leadership
• Focus on direct patient care and related administrative responsibilities
Required Qualifications
• MD or DO from an accredited program
• Active and unrestricted medical license (or willingness to obtain in state of practice)
• Willingness to obtain regional state licensure as needed
• Board Certified or Board Eligible (Family Medicine, Internal Medicine, or Geriatrics preferred)
• Active and unrestricted DEA license
• Medicare and Medicaid provider numbers
• Strong interpersonal and communication skills
• Commitment to a team-based care model
• Willingness to adapt to a value-based care environment
• Basic computer and EMR proficiency
• TB screening required upon selection
Preferred Experience
• HEDIS / STARS / CAHPS / HOS knowledge
• Medicare Advantage Global Risk Model experience
• Experience in medically underserved or culturally diverse communities
• Familiarity with electronic health records
• Comfort working within value-based reimbursement models
