• Serve as a vital liaison between clients, their families, and outpatient service providers to ensure cohesive care plans.
• Engage with insurance companies to navigate pre-certification processes and manage utilization requests, advocating for client needs and fostering partnerships for better care.
• Involve yourself in comprehensive discharge planning and aftercare follow-up to support clients' transitions and ongoing recovery.
• Provide therapeutic meal support to clients, modeling a healthy relationship with food in a safe environment.
• Deliver personalized care through empathetic listening, affirming support, and culturally competent practices tailored to each client's unique intersecting identities.

As an LCSW with Adelante Life Solutions, you will primarily work with a diverse client population, including those undergoing recovery and rehabilitation after substance use or behavioral health challenges. Your support will be critical for older adults and individuals grappling with complex emotional landscapes. Understanding the nuanced backgrounds of our clients will be essential for delivering care that truly resonates and meets their evolving needs.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of New Jersey is mandatory.
• Experience working with older adults and skill in addressing age-related mental health issues are preferred.
• Demonstrable clinical and diagnostic skills are essential for effectively assessing and responding to client needs.
• Strong communication and interpersonal skills, with the ability to build trusting relationships with clients and families.
• A proven capacity to work both independently and collaboratively within a multidisciplinary team structure.

While essential qualifications include a Master’s degree and an LCSW license, preference will be given to candidates with a rich background working within detox centers or similar environments. Familiarity with community resources, as well as experience in therapeutic settings that emphasize empowerment and recovery, will significantly enhance a candidate's suitability for this position.
