TLC Nursing Associates, Inc.
TLC Nursing Associates, Inc. is seeking a highly skilled and motivated Physical Therapist (PT) to join our Acute Care team. The PT will assess, plan, and implement individualized physical therapy programs to help patients restore mobility, strength, and independence following surgery, illness, or injury in a hospital or acute care setting.
Job Responsibilities
  • Perform comprehensive evaluations of patients' physical function, mobility, balance, and strength.
  • Develop and implement evidence-based treatment plans tailored to patient goals and clinical needs.
  • Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions.
  • Educate patients and families on safe mobility practices, home exercise programs, and injury prevention.
  • Collaborate with physicians, nurses, occupational therapists, and other interdisciplinary team members to ensure coordinated care.
  • Assess patients for equipment needs such as walkers, canes, or wheelchairs and recommend appropriate devices.
  • Monitor and document patient progress, outcomes, and response to treatment accurately and timely in the electronic health record (EHR).
  • Comply with facility, state, and federal regulations including safety, documentation, and infection control standards.
Qualifications
  • Current Physical Therapist (PT) license in the applicable state required.
  • Doctorate or Master's degree in Physical Therapy from an accredited program required.
  • BLS certification required.
  • Previous experience in an acute care, hospital, or rehabilitation setting preferred.
  • Strong understanding of anatomy, kinesiology, and evidence-based rehabilitation practices.
  • Excellent communication, critical thinking, and time management skills.
  • Ability to work collaboratively in a multidisciplinary healthcare environment.
  • Commitment to delivering compassionate, patient-centered care.
