Case Manager Registered Nurse | $5,000 Sign On Bonus

Role Overview:

We are seeking a Motivated Registered Nurse to join our team in Grand Junction as a Case Manager to assist with Private Duty Nursing Cases, Skilled Nursing Visits and the exciting new Market Growth and Development we are looking to make in the Western Slope.

***$5,000 Sign-On Bonus!***

Pay:

  • $87,000–$93,000 annually, with flexibility based on experience and overall fit

Schedule:

  • Full-time

Key Responsibilities:

  • Provide private duty nursing care to patients in their homes to help with caseload growth until full to continue in a supervisory capacity.
  • Provide skilled nursing visits to establish census.
  • Administer medication, treatments, and therapies as prescribed by the physician.
  • Monitor patient's condition and report any changes to the healthcare team.
  • Assist with activities of daily living and provide emotional support to patients and their families.
  • Collaborate with other healthcare professionals to ensure optimal patient care.
  • Maintain accurate and complete medical records using web-based electronic medical records system.
  • Assist with marketing and market growth development in Grand Junction and the surrounding areas.
  • Partner with talent acquisition and patient alignment.
  • Attend careers fairs, partner with local universities and attend various marketing opportunities as needed.

Requirements

  • Active RN License.
  • Previous Home Health experience required.
  • Current BLS Certification.
  • Valid Driver's License and Vehicle Insurance.
  • Strong time management and prioritization skills.
  • Excellent communication, assessment skills and attention to detail.
