Job Summary:
Under Article 29-I, the Licensed Behavioral Health Practitioner (LBHP) is responsible for providing behavioral health services, skill-building interventions, and psychiatric support services to youth in foster care and their families.
This role works within a multidisciplinary team to ensure that all mental, emotional, and behavioral health needs are met, while supporting permanency planning, family stabilization, and overall well-being.
The LBHP provides direct clinical support, collaborates with internal and external providers, and plays a key role in ensuring timely, accurate documentation and high-quality service delivery across all departments.
Duties and Responsibilities
Behavioral Health & Clinical Services
  • Provide behavioral health supports, including:
    • Individual counseling and treatment
    • Family and group counseling
    • Transitional counseling
    • Substance abuse screening and counseling
  • Deliver psychoeducation and wellness education to youth and families.
  • Conduct all annual and required assessments per Article 29-I guidelines.
  • Perform ongoing risk assessments and regularly evaluate safety and well-being.

Skill-Building Interventions
  • Provide structured skill-building sessions for youth and foster parents, including:
    • Communication skills
    • Problem-solving skills
    • Stress management
    • Anger management
    • Emotional regulation
    • De-escalation skills
    • Safety planning
    • Suicide risk assessment

Collaboration & Case Coordination
  • Maintain weekly, bi-monthly, or monthly check-ins with youth and foster parents based on risk level.
  • Obtain documentation and updates from psychiatrists, external therapists, and collateral providers.
  • Communicate consistently with:
    • Foster Care Department
    • Health Services
    • Health Homes Care Management
    • Intake Department
    • Education Department
    • Coaching Department
  • Provide referrals for additional supports, including CFTSS, HCBS, Early Intervention, and CPSE programs.
  • Participate in interdisciplinary and interdepartmental case conferences and treatment planning meetings.
  • Support integration of care across all departments and external agencies.

Program & Compliance
  • Ensure quality assurance and excellence in service provision.
  • Provide crisis intervention services as needed.
  • Complete all required documentation in a timely and accurate manner using EHR systems (Accumedic preferred).
  • Contribute to ongoing program improvement initiatives.
  • Perform other duties as assigned within the scope of the position.

Requirements
Skills, Knowledge & Abilities
  • Strong clinical assessment, diagnostic, and counseling skills.
  • Excellent verbal and written communication abilities.
  • Proficient in Word, Excel, EHR systems, and care coordination tools (Accumedic preferred).
  • Strong organizational, time management, and prioritization skills.
  • Ability to collaborate effectively within multidisciplinary teams.
  • Knowledge of managed care systems, Article 29-I requirements, and behavioral health regulations.
  • Ability to work independently while meeting program expectations.
  • Ability to perform essential job functions with or without reasonable accommodation.

Qualifications / Requirements
  • Master's degree required.
  • Active New York State license: LMSW, LCSW, LMHC, or LMFT.
  • Minimum 2 years of clinical experience in a mental health or behavioral health setting.
  • Must be able to work Monday-Friday in person for the first 90 days, with availability between 9:00am-7:00pm based on program needs.
  • Ability to attend required meetings, trainings, and case conferences.
  • Valid driver's license preferred but not required.
  • Required to travel to both sites (BK & S.I) as needed but we're flexible on where the position is based
