Matrix Providers is hiring a Nurse Practitioner – Primary Care to join our team of talented professionals providing healthcare services to Military Service Members and their families at Naval Branch Health Clinic Saratoga Springs, NY.

  • Employment Status: Full-time
  • Compensation: This is an hourly position, paid bi-weekly $72.20- $76.00/hr
  • Schedule: Monday to Friday 7:00 am to 4:00 pm

Requirements:

  • Degree: Master's degree in nursing.
  • Education: Graduate from an academic program, within the nursing specialty field of study that is accredited by a regional or national nursing accrediting agency recognized by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
  • Certifications:
    • Certified in the specialty required by the TO as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners (AANP) or the recognized national nursing certification organization for the specialty area of practice as appropriate (for example, National Certification Board of Pediatric Nurse Practitioners and Nurses, American Nurses Association, National Certification Corporation for the Obstetric, Gynecologic and Neonatal Specialties).
    • Basic Life Support through the American Heart Association or American Red Cross.
  • Experience: 12 months of experience as a nurse practitioner in outpatient primary care within the last 24 months.
  • Licensure/Registration: Current, full, active, and unrestricted license as a Registered Nurse.

Job Summary:

  • Provide comprehensive patient care, including assessment, diagnosis, treatment, preventive care, health education, and counseling within the scope of clinical privileges.
  • Conduct routine, acute, and chronic care evaluations, physical examinations, immunizations, family planning services, and patient follow-up.
  • Perform telephone triage using established protocols and coordinate appropriate ambulatory or emergency care for beneficiaries.
  • Develop and implement individualized patient care plans, promoting self-care and collaborating with patients, families, physicians, and interdisciplinary healthcare teams.
  • Accurately document patient care, maintain medical records, and utilize electronic health record systems and other DoD clinical databases.
  • Adhere to infection prevention, patient safety, emergency response, and risk management protocols while maintaining compliance with legal and regulatory requirements.
  • Participate in quality improvement initiatives, peer reviews, performance improvement programs, and patient care evaluations to enhance clinical outcomes.
  • Provide education, clinical guidance, and in-service training to staff, students, and support personnel while promoting evidence-based practice.
  • Maintain professional competency through required certifications, continuing education, annual mandatory training, and compliance with command policies.

Communicate effectively with patients, families, supervisors, and healthcare team members while exercising professional accountability, appropriate delegation, and timely reporting of clinical concerns.
